Google Forms
Google Forms is a data collection and assessment tool which can be used as either an online questionnaire or an online quiz respectively.
HOW TO CREATE AND GRADE A QUIZ
- Open Google Forms and create a BLANK FORM or use available TEMPLATES
- In SETTINGS, tick MAKE THIS A QUIZ
- In the QUESTIONS tab, add your questions and select the type of question you are intending to ask:
- Short Answer (Open Ended)
- Long Answer (Open Ended)
- Multiple Choice
- Checkbox (Multiple Answers)
- Dropdown
- File Upload
- Linear Scale
- Multiple Choice Grid
- Checkbox Grid (Multiple Answers)
- Select the correct answer in the ANSWER KEY, and add explanation in the FEEDBACK
- Adjust the POINTS per number
- If helpful, ADD sections to divide the entire quiz into topics
- Customize by adjusting the THEME
- In the SETTINGS tab, select whether the checking of the quiz or release of score and answers will be every after the student take the quiz or manually depending on the teacher's prerogative
- Share quiz by clicking SEND
- Check student performance in the RESPONSES tab
HOW TO CREATE ONLINE SURVEY FORM/ QUESTIONNAIRES
- Open Google Forms and create a BLANK FORM or use available TEMPLATES.
- In the QUESTIONS tab, add your survey questions and select the type of question you are intending to ask:
- Short Answer (Open Ended)
- Long Answer (Open Ended)
- Multiple Choice
- Checkbox (Multiple Answers)
- Dropdown
- File Upload
- Linear Scale
- Multiple Choice Grid
- Checkbox Grid (Multiple Answers)
- Add validation if needed
- Tag if the question is a REQUIRED question.
- If you have MULTIPLE SECTIONS, you can direct respondents to another section using conditionals and skippings.
- Share survey questionnaire by clicking SEND
- Check completed respondents in the RESPONSES tab
